Application for temporary residence visa (IMM 5257)
Family information form (IMM 5707 or IMM 5645) must be completed by those aged 18 years or older.
Use of representative form (IMM 5476)- Complet this form only if you use the service of a representative, or you are appointing or canceling a representative.
Two recent color photographs white background (35X45) write your name and date of birth in back.
Fee payment in an acceptable format, ensure
Valid passport (the validity of the passport has to exceed the end of the proposed trip by at least seven months. Make sure the passport has at least two blank pages left for visa purpose).
Clear photocopy of passport identification page.
Statement and solvency of personal and company bank account for the past 6 months.
Leave letter of employer (with address and telephone number) stating duration of granted leave.
Letter of the applicant’s university / school granting the intended leave
Proof of personal living conditions: marriage certificate, children’s birth certificate(s) (if applicable).
Flight reservation copy.
Tour plan (itinerary) for the entire travel with mention of names and addresses of all the hotels or places you plan to stay.
